Supported Transitions: Our Tier 4 Discharge Model
Our Tier 4 Discharge Model is a comprehensive, tiered support framework designed specifically for neurodivergent young people who have been sectioned under the Mental Health Act. Providing a clear, compassionate pathway from inpatient admission through to community reintegration, with a strong focus on continuity of care, family involvement, and sustainable recovery.
This model is built on a specialist neurodivergent-informed approach, shaped by lived experience and clinical expertise. It ensures that young people and their families are not left to navigate the transition alone, instead, they are supported by a consistent team, with interventions that are responsive, relational, and person-centred.
This package of support offers real, measurable outcomes for everyone involved, from young people and their families to the professionals and commissioners working around them. It’s designed to create stability, reduce risk, and build confidence at every stage of the transition from inpatient care to community life.
